So this is a Dr. Kyrsten McWilliams. She's an audiologist and owner of the hearing solution, and she's going to talk to us about what it's like to be in a private practice and what it's like to own one. Yeah, it's exciting stuff. I made a little PowerPoint, I'm going to share. Um,

private practice. So I want to tell you first about like my history, because it's not like, I don't think I was the only person in my class with history. That was I already had my hands on hearing aids for most of my life because my mom, my mom is a audiologist. So there we go. Yeah. So the original Dr. McWilliams. She, my mom did her externship and Humboldt. She went to Humboldt State, and then opened her private practice in Redding and 1985. So I love this. This is I like redid the picture of my mom in her externship at age 27. And we did mine at age 30. It's really funny, but she just actually retired this year, she finally found somebody to buy her practice. But I did not take over her practice, because I did not want to move back to reading.

But a little bit about like the history there. My mom's practice was, it was really just her and her receptionist, she started started it by herself.

And then eventually, she, like I did some billing, I did some bookkeeping, I would do janitorial. I eventually became an assistant. And then I got my license as a hearing aid dispenser. And I was dispensing there for about four years before making the decision to go and pay the money and get my degree.

Which, you know, she, I think, thought that I was going to go back and take over her practice. And that did not happen. But it all works out. She found she found somebody from Pacific University to buy her practice last year. Very exciting. Um, so my undergrad degree was actually a BS in psychology from UC Davis. And then I had that four year kind of break, where I was the dispenser with my mom before deciding to go back to grad school. So I was a little older than the rest of the people in my class definitely gave me kind of a leg up in that I already knew everything about hearing aids, like changing wax traps, how to program them. So I was really able to spend a lot of time focusing on things I didn't know about like tinnitus and cochlear implants. So this year, we actually integrated cochlear implants into our practice. And that's like a whole other piece, but it's neat, you know, you can, you can do everything in practice in private practice, or you can pick and choose what you want, like some people will do visted I don't like this tip. And so I can choose not to do that.

So, I was trying to think of like they, what would I want to know, back when I was a student, I guess, get as much private practice experience as you can and like absorb as much as you can, you know, in the clinic. But also, I remember being assigned a lot of assignments in grad school that were like, create your own oral rehab program or create handouts for a patient for tinnitus. And I use all of the things that I created in grad school. So when you have an assignment like that, like really thinking about like, you can potentially be using this on your patients later on. It was helpful. So they haven't changed at all. I still have them and use them every day.

And then when once you know after grad school, getting out there into the real world.

The there's so many, like I don't I guess like audiology got started really in the 80s. And so you have to think all of these people didn't have practices that they could go buy from another audiologist. These these audiologists were actually at the time they weren't even allowed to dispense hearing aids that like came about later. They were they could only do audiology. So once they were allowed to start dispensing, hearing aids, and they started there, they had to all started their private practices from the ground up just like my mom.

And now they're getting to the age where they're all starting to retire. So, like getting a practice of your own

We'll probably be very different than just starting from scratch, because there's so many out there that are, are looking to retire, like

I know a lot, or all you have to do is wait.

It won't be long before they do. Or partner with one, you know, I partnered here at the hearing solution, Julia Tanner was the audiologist. And she started the practice about 20 years ago. And I came on with her for about three years before for the three, four years, as we kind of did the transition of changing ownership because it's, it's a lot, but you don't have to know anything about it. Because there's a lot of help in

these kind of groups that are that work with audiology practices. So

they're called buying groups, essentially, I don't know if you guys know how much you know about this kind of stuff. But there's some out there, I partnered with a buying group called fuel medical, and what they pretty much do, they've helped me facilitate the entire transition of the practice and the purchase, you know, reviewed the purchase agreement, the letter of intent, they did all of that, like figuring out pricing all of this stuff, or me didn't have to know any of that, because that's not my specialty, right? Like you let somebody else do that. That's their specialty. But you want to make sure you partner with somebody that you trust, and you know, is going to have your back because you'll actually be working with these companies for like my loan is 10 years, so at least 10 years, I'll be working with them. And then as far as funding goes, you know, I don't have a ton of money in my bank account to just go buy a practice up the street, not that fortunate. But manufacturers will actually loan you money to buy a practice. And for the most part, they're no interest. So you want to decide on a manufacturer that number one, you get the best terms with terms being

how much you're going to pay for hearing aids, your like cost of goods, and then you have a

manufacturer that you really like believe in and you trust is going to like, kind of stand the test of time, and be something that you're proud to like advocate for saying that, like you believe it's, it's a good product.

So on saying you guys can ask me, like I'm an open book, if you have any questions about any of this stuff, like I, I'm not gonna hide anything. Um, so the, the kind of why private practice is the best out of everything out there. It's because you get to pick and choose, you get to do everything you want to do, you get to spend as much time with each patient as you want, you can leave early if you want to, you can take pick a lunch break that you want, you can

do whatever you want. And, and you really get to,

will give you I think the biggest part is if you know what your y is, if you know what your mission statement is, and like what you want your practice to stand for, then all of the rest is very easy. Because you train your, you know, you get everybody your staff on the same page that you're all kind of working towards the same why and so everybody's on the same page. And, and then it's easy, patients just love that. And then they send other people, they send their friends, doctors, you know, refer patients to you, so you don't have to do very much marketing.

Actually, for the most part, we don't do like a ton of marketing, it's mostly word of mouth and physician referrals.

Um, and then, you know, it's a great leadership role. I never like I always knew I was going to own a practice, but I didn't realize that the leadership role would be so much fun.

The picture, I was actually a picture of my grandparents, I just thought it was adorable, when they're in memory care together. And that that's like part of my why, you know, like, these little they need, they need our help.

And we want to make sure they get the best care that we can get them.

So well. So some challenges, you know, you sure you get to set your own schedule, but you also have to work a lot. So that means I sometimes if my schedule is really really busy, I will schedule patients on a Saturday or late at night. cochlear implants take a ton of time. So scheduling those at the end of the day because they go longer than you expect to always.

Then there's also you know managing employees and payroll and books and everything. You don't have sick days like I have a cold right now but no I don't get

to stay home, you work unless you physically cannot, cannot work.

And then when you go home, there's always something that does need to be done. But you kind of get to pick and choose when you want to do that. So what you do, there's a lot of homework, I guess you could say. And then balancing your your work life balance, because that gets in the way too. So finding a partner probably that understands that you do have to bring your work home, because it becomes part of both of your lives.

I think like, anybody going out there and getting into private practice for the first time is going to be partnering with one of these, they are like vultures, they will all come after you and like try to get you on their, their team. I

I like interviewed and worked with about three of them before making a decision, just to kind of get a vibe of what they're about, like, what's their mission statement?

What they're really yeah, they all have like different kind of agendas. Like, and you guys, can I ask, you know, if you take notes of my like contact information and want to talk to me like a year from now, or two years from now or whatever about these things, I'm happy to do that. Because when you talk to the manufacturer reps, or when you talk to the buying group reps, they're all just in it for themselves, it really is

And then when once you have your practice, you get to decide how you want to collect the money, you know, you're not just working for an EMT who's doing all the billing for you, you kind of have to do that on your own. So there's practices out there that either bundle their services or don't bundle, which means, you know, when you hear about hearing aids that are $7,000. That's because all the services are bundled, you guys probably already know all of this. But you know, we hear do bundle, because that's

kind of what our mission statement is, I suppose it's like you sure the hearing aids are very expensive, but you get what you pay for. And so you know, you're going to be taken care of for the rest of the years to come on that service agreement. Versus I know other places, maybe more rural areas or, you know, 10 To button, they do not bundling non bundling, I've seen a lot of schools do non bundling to.

Yeah, let's see. And then there's managed care to get into managed care or not. It's a really hot topic right down there. I don't know if you guys know, like true hearing UnitedHealthcare. Hearing no, none of these things.

So pretty much there. You know, you have the manufacturer and the manufacturer that back in the day there, we did not have these, we call them I guess buying groups as well. But we didn't have these third party payers, we would just buy hearing aids from the manufacturer, and then they would go to the patient. But now these

third parties, I guess, have have come that have been created, where they are the ones who buy the hearing aids from the manufacturers, and then they provide

the provider, they send the patient to that providers office, whoever's in their network, and they provide the hearing aids and then they reimburse the provider like pennies compared to what you really need to do.

For the amount of time that you really need to spend with a patient. And if you're gonna give like good care, the reimbursement is just not there.

Okay, yeah. There's a lot that goes into that. It's hard, because I don't know how much you guys know already. But how do you handle that in your practice? Like you take care party? No, we're strictly no third party. But that's because we can, you know, I mean, a lot of places and I follow all these groups, you know, on Facebook, and in other states, like 30% of their patients are third party. So I think the guideline right now is if you're booked out two weeks, then and two or three weeks, then you're good, not taking third party. But if you're only booked out a week, and your schedule really can support these extra patients, then you know, minus, we'll be making something versus if you have an empty schedule, you're not making any.

But yeah, it's just it doesn't go. You know, going back to the mission statement, accepting these third parties would not go with our mission statement, which is, you know, to provide the best care possible and take the time that's truly needed. So with managed care, I don't think you'd have like time for real error or

But another kind of group that you can dip into are veterans programs. So some QTC L, HIV s, those are I think the top ones, pretty much the VA is overloaded, and they don't have the

bandwidth to take everybody that needs to be tested, these are for like Compensation and Pension Exams. So you sign up with a program and then they will refer the patient to you to do the hearing test. And then you have to like kind of fill out therefore and and you get paid some that and so we do a little bit of that when our schedule is empty, because that that will take up some slots and you can make money.

so I wanted to just stress like how important the team is like your your people.

You know, starting out like usually, my mom only had one person, a lot of people only have one person, or they'll have like their spouse as the receptionist or helping out and things like that. But as you grow I would definitely recommend at least one audiology assistant per audiologist.

We're we're trying to work on

getting like, our audiology assistants do so much for so much so much. They're really critical. And it just takes a lot of like the time that they need so much times patients need so much time spent with them when they're going through this process for the first time. And they're scared and they feel vulnerable and a little bit nervous. And so for us to actually like take the time and make sure they're feeling very confident by the time they leave the office. Then they don't go home and say oh yeah, nobody showed me how to change my wax trap. Like they know how to change their wax. They know these things like they're empowered. Um, oh, going to going down my little. In this photo on the very far left, we have Sandy. She's an audiology assistant. And then we have Denise she's our front desk. And Elizabeth Barragan you guys probably know Dr. Barragan. Me. And then Tanner is the last one. He's also an audiology assistant. So I love them. And then we also need you need a billing specialist in a bookkeeper. And those those positions are really hard to find. It's important to find one that is that you trust you really trust especially the bookkeeper, billing specialist, I know that the

buying groups like you know, fuel, cute QC partners, all those buying groups that we kind of touched on, they can get you connected with billing specialists. So if like, you're getting started with a practice and you don't have a biller, which is a critical part of your team,

you may be when you're shopping around with those buying groups looking to see if one of them can connect you with a, a billing specialist. And if they have a good reputation or not, because we've tried

like a remote Biller, and it was it was terrible, didn't work out. So making sure you do your research and not just talking to the reps, but like actually talking to other audiologists who do use them and what their experience has been

Yeah. So look, yeah, certifications are needed for an audiology assistant. Oh, my gosh, this is a hot topic right now too. Because it's like, there's really none. You have to pay like, I think $10 and do fingerprints. And then write a description as to what they're going to be doing in the office. So all of the liability for that license really falls on the audiologist. Anything that you write down that that assistant is going to be doing you better feel very confident that they know how to do those things. I will say just from experience that your license will be denied if you write down anything that

Oh, yeah, I can read it. Are there things that we can do in grad school to better prepare for owning or leading a private practice. Um, I really feel like when I when I like looking back, because I was I was just there not that long ago, I graduated in 2018. And I just remember being very grateful that I spent so much time on those like handout things, because the information you have in your brain is so fresh. And when you get out in the real world, you lose so much of it. So if you can, like take what you're learning and digest it into a way that is really easy for like a patient to understand.

It's really helpful. Coming up with content, we do our own blogs, we do our own like stuff on the website, and it's just a lot of content. So save, save any content that you feel like could be used, like on your website, or things like that later on.

You know, because I know that a lot of Pediatrics happen in the hospital setting. And

I guess, because I have a son who is deaf and hard of hearing. I don't think we've ever sought out pediatric audiologists, you know, we kind of just stay in the hospital, also, because we're with Kaiser, but you know, when we were thinking about changing into insurances, we didn't hear about any audiologists, like when we were asking friends that are not in the Kaiser system, like, you know, who do you go see, and it was always kind of still tied to like UC Davis or like a senator or a hospital. So

yeah, do you? Do you feel that there's a need for private practice for pediatrics? Or is that still should just remain in the hospital setting? Well, I guess it I mean, I totally think there's a need, it just depends on like, what percentage of the population is there a need for it because you have to think in private practice. And this is the whole reason why pediatrics is mostly in medical models, because the billing, the reimbursement isn't there. So you can bill for some things, but for the most part and just like cochlear implants to you can bill for some things, you're not getting reimbursed very much, but really, you're having to compensate with asking somebody to give you money. So these are private pay patients.

Like my mom did a ton of Pediatrics but she was also in network with medical and CCs and so got all of those referrals and there's a ton of kids out there who need that but that was just you know, up and running. It's it was different.

Oh my gosh, it took me months to go through all of them I even wanted, it was like all through COVID to I wanted to go up and like see their offices and, like, see what they were like in their little poems, but I didn't get a chance to do that.

It's really hard because they're also Volturi. And like, they tell you what you want to hear. And

it's just crazy because they're making so much money off of like the work that they actually do for you. You know, ideally you have like a ton of work that they can do. The problem that I have with buying groups is like their marketing, their website stuff, their handouts, it all is really like bland or they all look the same to me. Like if you look at Med PB is a company that makes audiology websites if you pull up a bunch of other offices, they all look the same.

So we I actually like pay a separate marketing group to do our marketing so that it doesn't look so

buying group fish. And so it's like I question myself, well, what is the buying group actually doing to work for me? And they helped a lot with the transition and that was invaluable. I've been

Are you like stuck in a contract with them for so many years? Or how does that work. So when you're doing your the purchase agreement, like make sure that you can get out of it, and you can switch. It's, you have to have a lot of patience. So much patience, um, a lot of patience. And I mean, you have to, in a way, be like, not a salesman, but you really are, you're asking people to fork over like $7,000 to wear something they really don't want to wear. And you have to make them believe that they want to, you know, you have to prove to them Yeah.

So it takes a little bit of that customer service, ask skill, but really, I think it takes just a lot of patience, because they move at a very slow pace. And you just want to make sure that they feel like heard and not rushed. And, like, like I said, like just confident that they know number one, that they're making the right decision going with you. And number two, that they like they know their hearing loss, they understand their hearing loss, and they understand their hearing devices.

It's like you don't get a, I remember working at UC Davis, and, you know, you got 45 minutes for an audio. And so you take that person back and you test their hearing, and then you say goodbye, knowing you're never gonna see them again. And it's just a totally different energy. When you're working with somebody that really, you're probably never going to see again. Versus like in private practice. As soon as you meet that person. Like, likely that'll be somebody you have until the day they die. And like you'll probably take their family members or their spouse and their siblings, and it's really that's the best part.

Yeah. And it's like in the beginning, when you have a lot of new ones and you're taking over I remember being very very overwhelmed with like, am I gonna remember their name? She just knew every little detail about their lives. But it was because she saw them every six months for 20 years. And it doesn't take long to like

Oh, gosh, yeah. Because it's been, it's been what, like five years. That's crazy. It feels like yesterday, grad school tips, your relationships are so important, because that's really how, like you build a connection, and then that person will come back and ask you to come back.

Yeah, I guess I don't know if do you guys have any, like specific questions? I don't know. Like, where where you're at? Like, specific, maybe more specifics?

maybe with like doing well, in internships, I feel like it's right now. We only do four hours a week. And so it's always kind of hard to feel confident in what I'm doing going because I do it for hours a week. And then I have a week off from it. And I'm like, What did I do? So how can you like overcome that impostor syndrome in like, the first round of clinic, I guess, practicing on each other. I don't know. If anybody's like, I would practice, we practice with our roommates and get together and do impressions and do a task fee or whatever, you know, all those things. We were in the clinic all the time doing, like hearing tests, and all that constantly. And then just knowing that, like, the supervisor knows that you're a student and doesn't expect you to know at all. In fact, if you go in pretending that you like thinking that you do that you're not there to learn then that we I mean, I've heard people have seen students like that and it's like, well, they're probably not going to get called back for an externship. So, yeah.

Um, gosh, there's so many practices out there, like the practices of private practices out there who are taking students like are doing really well.

Like a practice has to be doing really well to have the confidence to take a student and know that, like, their business won't be affected by that in a way.

At the same time, if they're just trying to use you as free labor, that's like I've heard of, like, crazy, bad places like that, too. But I don't think I think like the person that connects you, I can't remember who your liaison is. They usually vet those like clinics out hopefully.

There's the with private practice, there's like huge clinics, and then there's small clinics. So I don't know if somebody's like super ambitious and wants to have like five locations, then maybe finding a clinic, like wherever that does have multiple locations. But at the same time, if somebody has multiple locations, I might be worried that they're going to send you to like the furthest one away that nobody wants to go to and you'll be there by yourself not really learning anything like about the clinic. You know?

I, what type of experience would you recommend, like getting while we're in school, before going to private practice

I will say like, hearing aid experience is really important. I feel like most of my cohort graduated, really not being truly like comfortable with hearing aids, like this did tinnitus, all this other stuff is great, but like actually programming and hearing aid, or knowing how to like troubleshoot a follow up.

Really, really weak in that. So yeah, we'll practice a lot with hearing aids because that's it. That's what private practice mostly is.

So it would be it's in a way considered like the coaching that we do in the how we set it up and make sure really what it is, it's like, having an assistant make sure that they can download the app and get the link to download the app, and they know their password to get it on their phone. But once they have it, and we do actually make them pay for amplify themselves. But that's because they wouldn't

probably go through the program if it was free. So a lot of and a lot of them do it because it's important. They understand that the hearing aids don't just fix everything. Yeah. And amplify has been really helpful with the oral rehab.

Did you guys ever do group auditory training? Or do you still do that? We haven't like since COVID. We haven't gotten back into doing groups. But we we want to, again, it's a space thing. We wish we could fit more people. We can only fit about eight or nine. But yeah, I want to and I want to start doing like a cochlear implant support group.
